Joining Forms

Prior to joining Leighton Park, parents and guardians are required to complete a variety of joining forms. These can be accessed via the Admissions Portal.

You will be asked to Sign In with your parent email (or agent account). The forms will appear as Checklist items. The OpenApply Parent App (which we call the Admissions Portal) is now available on iOS (requires iOS 14.0 or later) and Android. 
 

Joining Forms

Mandatory Forms which must be completed for all students:

• Student and Parent Information - Day students will need to include details of an emergency contact which must be DIFFERENT from the parent details. Boarders will require details of their guardians
• Medical Information and Consent - you will be asked to list your student's immunisation details. These can be requested from your GP
• IT Acceptable Use Form (for Years 7 - 11 and Pre-Sixth)
• Language and Subject Choices form
• Easy4U device order (for Years 7 - 11 and Pre-Sixth)

Boarding Forms:

• Guardianship Agreement

Optional Forms (to be completed where applicable):

• Individual Learning Centre (ILC) Form for students wishing to have individual learning tuition
• English as an Additional Language (EAL) Form for overseas students
• Musical Instrument/Voice Tuition
• School Transport
• Orientation/Induction Day attendance
